Essays and Other Required Documents for CHP Scholarships
The following information and instructions are provided to assist students in the preparation and submission of essays and other required documents to include with their CHP scholarship applications.
NOTE: Deadlines to submit scholarship applications and any associated documents will vary. Please check ScholarshipUniverse for scholarship/fellowship deadlines.
ScholarshipUniverse Submissions
For all CHP student activity charts submitted in ScholarshipUniverse please include:
- Organizations: social, professional, and/or academic with offices and leadership roles
held
- special honors
- community service
- academic research
- work experiences with population worked with (e.g. hospital: neonatal department; clinic: elderly 60+)
Financial Need
Several scholarships at 成人头条 State require students to demonstrate need. If your financial circumstances change significantly after submitting your online application, please contact the Office of Financial Aid to discuss any financial concerns you may have.
- Financial need is officially determined by the results of the Free Application for Federal Student Aid (FAFSA).
- A current FAFSA on file is highly recommended prior to submitting scholarship/fellowship applications. In some extenuating circumstances, financial need can be established through a financial need statement and supporting documentation.
- If you are a US Citizen or permanent resident, you must complete the FAFSA for the 2022-2023 academic year at .
- If you are ineligible to apply for Federal Student Aid via the FAFSA (such as an undocumented student, DACA student, or international student), provide a statement and attach supporting documentation in the 鈥淔inancial Need鈥 section of the Scholarship Universe application.
- If you have extenuating circumstances impacting your ability to pay for college that are not reflected on your FAFSA, provide a statement and attach supporting documentation in the 鈥淔inancial Need鈥 section of the Scholarship Universe application.
Essay Instructions for the Financial Need section of the ScholarshipUniverse application:
Please provide a 250- 500 word (no more than 3,000 characters) typed essay describing your family's financial circumstances and financial need. Details are important as this statement may be used to consider you for need-based scholarships.

The following are essay and other document requirements that are specific to each department.
Communication Sciences and Disorders
All CSD scholarship/fellowship applicants:
- Must complete the essay section in the Scholarship Universe application.
Use this section to highlight your special interests, talents, leadership qualities, background, unique experiences, and long-range professional goals. - Applicants will email their additional scholarship materials to csd@wichita.edu by the designated deadline in Scholarship Universe.
BARBARA WILLIAMS HODSON SCHOLARSHIP IN COMMUNICATION SCIENCES & DISORDERS - 611073
- Submit two (2) recommendation letters from CSD faculty members.
- Other scholarship guideline requirements are entered in Scholarship Universe and submitted in the activities chart and/or matching data.
KENT AND JANET KRUSKE FELLOWSHIP IN SPEECH-LANGUAGE PATHOLOGY 鈥 611052
- Submit a 500-word, double-spaced, typed essay describing how you anticipate capitalizing on your practicum experience and your long-term career goals.
Dental Hygiene
- Documents may be submitted to the Dental Hygiene Office located in Ahlberg Hall Rm 312 Attn: Karla Sharp by the designated deadline in Scholarship Universe or emailed to dental.hygiene@wichita.edu
Diane E. Huntley Scholarship 鈥 611170
- Submit a one-page, double-spaced, typed essay explaining your concept of patient-centered care, your involvement in professional organizations, and community service.
Kansas Dental Hygienists Association -610559
- Submit a one-page, double-spaced, typed essay describing:
- Your reason for applying for this scholarship. Stating your goals for your dental hygiene career, your relationship, and goals with your professional dental hygiene organization.
- Describe how you currently demonstrate professionalism and your goals for continual growth in this area.
- Explain why you believe the Kansas Dental Hygienists鈥 Association should award this scholarship to you.
- Consideration to students who submit a written statement of community involvement.
NOTE: A submitted activities chart in your Scholarship Universe application fulfills this requirement.
Kari Rinehart Memorial Scholarship 鈥 611279
- Submit a 500-word, double-spaced, typed essay describing the qualities necessary to perform as a dental hygienist and how these qualities relate to you personally.

- Provide proof of current membership in the WSU Chapter of the Student American Dental Hygienists Association (SADHA) by submitting your ADHA membership number and/or membership approval e-mail from ADHA.
- Consideration to students who submit a written statement of community involvement.
NOTE: A submitted activities chart in your Scholarship Universe application fulfills this requirement.

- Must intend to practice dental hygiene at least three days a week in Kansas continuously
for at least two years immediately following completion of the program and sign a document attesting to this intention.

Medical Laboratory Sciences
CRG Scholarship - 211305
- Provide an essay describing how you plan on using your Medical Laboratory Science degree after graduation/how a student can use a Medical Laboratory Science degree outside of a clinical setting.
- Essays will be turned into the Chair of the Medical Laboratory Sciences program.
- The essay should not exceed one page, be double spaced, and in 12-point font.
- Essays may be submitted to mls@wichita.edu or delivered to the MLS office located in Ahlberg Hall Room 107 by the designated deadline in Scholarship Universe.
Nursing
All Nursing Scholarship/Fellowship Applicants Must Submit:

A Personal Statement
In the essay section of your Scholarship Universe application please describe your leadership qualities, why you are pursuing a nursing degree, and your aspirations in serving as a nurse. -
One Letter of Recommendation
- A letter of recommendation must come from a current or previous employer, instructor, volunteer supervisor or coordinator, etc.

- Must have intention to work in Labor and Delivery and/or Neonatal Nursing

- Submit a short essay describing why you are pursuing a degree in nursing.
Patricia J. McNamara Huntington Memorial Endowed Scholarship 鈥 610093
- Applicants will submit a 750-word essay outlining:
- why you wish to purse a degree in nursing and
- what your aspirations are in serving as a nurse.
*Essay requested for all nursing applicants fulfills this requirement.
Vicky S. Brookhouser Memorial Endowed Scholarship 鈥 610297
- Submit a one-to-two-page biography outlining:
- what motivated you to return to school and enter the nursing profession and
- what can be identified as special life circumstances/occurrences which motivate application for this scholarship.
Physician Associate
- Provide an updated copy of your Student Activity Chart
- Provide a double-spaced, typed essay (200 to 500 words) describing each of the following as applicable. If applying for multiple fellowships, please submit all essays as one document with each essay on a separate page:
- Scanlan, Lary, and/or Gates Fellowships: Discuss your definition of leadership and community service, why you are passionate about it, and highlight any activities you wish
- KAPA Fellowship: Discuss your ties to and passion for Kansas and your future career goals as a Kansas PA
- Shaw Fellowship: Discuss your passion for primary care and future career goals as a PA working in primary care
- McKinney Fellowship: Discuss how your EMT training will make you a better PA and your commitment to healthy living
- Submit the Student Activity Chart and Essay document via email to Evan Ohlman evan.ohlman@wichita.edu on or before July 1st.
Physical Therapy
No additional documents or essays requested outside of the Scholarship Universe application at this time.

Public Health Sciences

Dr. Amy Drassen Ham HEALTH Student Association Scholarship
- This award is contingent on becoming an active member of the HEALTH (Health Education Advocates for Leadership, Teamwork, and Humanity) Student Association through providing a minimum of 10 hours of service per semester to the association during the academic year of award.
- Submit an essay describing your plans upon graduation, how membership in the HEALTH Student Association will enhance your education, the importance of this support for sustaining your education, and details of your commitment to improving health education and promoting diverse care.

College of Health Profession's General Scholarship Instructions

- Applicants must provide a letter of recommendation from one service organization with which the student has volunteered.
- Submit your letter of recommendation to the College of Health Profession鈥檚 Dean鈥檚 Office located in Ahlberg Hall Room 400 or healthprofessions@wichita.edu by the designated deadline in Scholarship Universe
Peter Cohen Scholarship
- Provide a one to two-page, double-spaced, personal statement highlighting academic, leadership, extracurricular, community service, and work experiences.
- Include any type of participation in health-related activities such as volunteering, observation, and patient care.
- Completed essays are to be emailed to fhcpwsu@gmail.com or delivered to Ahlberg Hall 400 by 5:00pm on October 1st.
